American Society for Healthcare Risk Management - Focuses on the role risk management plays in patient safety. The Anesthesia Patient Safety Foundation - Seeks to ensure patients are not harmed by the effects of anesthesia. Aviation Safety Reporting System - a cooperative program established by the Federal Aviation Administration's Office of the Assistant Administrator for System Safety, and administered by NASA. It is a good example of an effective solution of anonymous reporting that has resulted in reduction of error.
